The eyelids are critical for the production of two important components of tears: the oily layer of the tear film (hyaluronic acid) and antibodies. The sebaceous layer is produced by the meibomian glands (composed primarily of collagen), which are 20 in number in both the upper and lower eyelid. The lower eyelids produce most of the tear film, and the upper eyelid has more of a “windshield wiper” function to evenly distribute the oil over the corneal surface. Hyaluronic acid has the ability to absorb oxygen and hyperoxygenate, thus supplying oxygen to the cornea by diffusion. Any abnormality of the tear film (too much water or too much acidity) will create hypoxia and slowly lead to corneal problems.
The second important function of the tear film is to prevent infections on the surface of the eye and eyelashes.
