How Fractional Peeling Works

Ecomed CO2 Ultrapuls emits a laser beam of 80 micrometers and a speed of 500 pulses per second. Through the Fractional Scanner II, it scans an 18/18 mm area in less than a second and vaporizes 400 to 1200 micron dots of epidermis while leaving the rest of the area untouched. Depending on the desired effect, the Fractional Scanner II can achieve vaporization densities from 49 (7x7) to 484 (22x22) dots per square centimeter. The higher the evaporation density (degree of ablation), the better the effect and the longer the patient's recovery time. Fractional resurfacing is an alternative to full laser resurfacing, in which the top layer of the epidermis is 100% vaporized.

What the patient feels during the procedure

As the laser beam vaporizes the micro dots from the skin, the patient feels prickles at high speed. Depending on the degree of ablation, the discomfort of the procedure increases.

What is the expected outcome of the procedure?

Fractional peeling vaporizes a portion - between 20% and 40% of the patient's epidermis and heats the skin in depth. The healthy / unaffected / part of the skin helps to quickly recover the affected. As a result, the damaged epidermis layer is replaced with a new one and new collagen synthesis is stimulated. The skin is rejuvenated, plumped, more elastic and youthful looking. Superficial wrinkles and skin blemishes are reduced.

What is the recovery period?

Depending on the individual patient and what part of the epidermis is affected, the recovery period can last from 24 hours to 72 hours. In the first few minutes to a few hours after the procedure, the skin is red and painful as after a sunburn. The use of moisturizer and sunscreen with SPF 50+ is a must. After this period of discomfort, the redness subsides and the patient can return to their normal activities provided they avoid sun exposure and use moisturizer.

How many treatments are needed?

Depending on the age of the patient, the desired result and the degree of ablation, 2 to 4 treatments are needed at intervals of 1 month, but results are seen after the first treatment.

How laser resurfacing works

Dermatological lasers emit a thin beam of light with high energy and density that vaporizes water in the skin and thus causes controlled damage (ablation) of the skin. Depending on the type of lasers and the wavelength they emit, different results can be achieved:

Total ablation

This is the classic version of the procedure, where the superficial layer of skin on the entire face is removed. It produces the most dramatic results at the cost of a long recovery period and significant risks of post-operative complications. The full laser peel uses a CO2 laser. Classic laser resurfacing is not used in Skin Line aesthetic clinics due to the availability of more gentle alternatives.

Fractional laser resurfacing

Here, with the help of CO2 , Er:YAG or ResurFX lasers, microscopic column burns are made in the skin with different depths and densities, according to the effect being pursued. Most commonly, 20% to 50% of the skin is damaged in fractional peels. In the recovery process, unaffected skin helps the skin damaged by the peel to recover. The surface layer of skin is exfoliated and new, smooth and soft skin appears underneath without imperfections.

How many laser peel treatments are needed?

The number of laser resurfacing treatments that are needed to correct an aesthetic problem depends on several factors:

To achieve a similar effect, but for more severe forms of pigmentation due to sun damage in a 60 year old female smoker who sees a sedentary lifestyle, 3 treatments of deep or ultra-deep fractional laser resurfacing with a CO2 laser will be required. For whom laser resurfacing is not suitable - contraindications

Laser resurfacing is a procedure that is not suitable for everyone and carries some risks. Laser resurfacing is not appropriate when you have active infections, viral, fungal or bacterial diseases, inflammatory skin conditions or skin cancer; Pregnancy or breastfeeding; Individuals with an electronic implant, such as a glucose monitor, pacemaker or defibrillator or a permanent implant in the treated area; Cancer or a history of cancer; Immunosuppressive diseases such as AIDS and HIV or use of immunosuppressive medications; Heart disease, epilepsy, uncontrolled hypertension, and liver or kidney disease; Herpes simplex in the treatment area; Active ulcers, psoriasis, eczema, or rashes; Uncontrolled endocrine disorders such as diabetes or thyroid dysfunction; Biologic fillers injected into the area within the past 6 months or Botox within the past 2 weeks; Use of isotretinoin (Accutane) within 6 months prior to treatment.

Recovery after laser resurfacing

Laser resurfacing in most of its varieties is an ablative procedure, which means that the integrity of the skin is compromised and it takes time for it to recover.

Risks may include: redness, swelling, scarring or pigmentation change. Some redness and swelling may occur immediately after laser resurfacing depending on the customized treatment settings. The swelling may subside after a few hours and the redness the next day or over the next few days. Most patients are able to return to their daily activities the same day.

Usually, the doctor should advise you not to expose yourself to direct sun for several days after treatment and to use sunscreen.

Here are some of the most common risks of laser resurfacing:

Redness, swelling and itching: These symptoms are usually mild and go away within a few days.

Dry or flaky skin: This can last for several weeks after the procedure.
